From the Introduction
A surgeon traces a path across a laminated drawing of the gut with the cap of a pen: this part stays, this part is removed, this part is joined much further down. Almost everyone in this book remembers a moment like that. The writers here describe choosing a rarer, bigger operation, and the years of vitamins, blood tests and bathroom planning that came after it.

About this volume
Inside this volume
Fifty first-person accounts describe life before and after a duodenal switch or SADI-S, the less common weight-loss operations that reroute much of the gut. Writers came to it at very high weights, or with type 2 diabetes and insulin, eleven years of sleeping in a mask, PCOS, a knee replacement on hold, a father's amputation or a sleeve that had stopped working. They include a long-haul trucker, a Queensland cattle farmer, a paramedic on twelve-hour shifts, a flight attendant, a vegetarian software engineer, a chef, a 24-year-old in IT support and a 71-year-old told he was too old, writing from Memphis to Mexico City and Aarhus to Adelaide. Their stories take in insurance appeals, paying by loan and credit card, surgery abroad, a two-stage operation, gas and urgent bathroom trips at work, the cost of vitamins, alcohol, antidepressants that stopped working, pregnancy, skin surgery and new prosthetic sockets. The harder accounts are told plainly: an emergency return to hospital on day five, a night of twisting pain on the bathroom floor, weight loss that went too far and led to one switch being lengthened and another partly reversed, reflux that stayed, binge eating that surgery did not end, and weight that returned in year six. Many describe ordinary movement coming back: walking a practice field, dancing, carrying a sofa upstairs.
Who it's for
For anyone weighing a duodenal switch or SADI-S, considering one after a sleeve, recovering from one, or living alongside someone who has had one and wants to understand the lifelong follow-up.
