For a century, type 1 diabetes meant doing the pancreas's arithmetic by hand — counting, dosing, correcting, guessing, around the clock. Closed-loop systems — an insulin pump and a glucose sensor with an algorithm between them, adjusting insulin every few minutes — have moved that arithmetic substantially into software, and the people wearing them describe the change in terms bigger than numbers.
What actually changes day to day
The accounts converge on nights first: the algorithm quietly heading off lows while the wearer sleeps, and parents of children with T1D describing the first stretch of unbroken sleep in years. Days follow — time-in-range climbing, fewer emergency juice boxes, the mental static of constant forecasting turned down. The phrase that recurs across testimony is some version of “I got part of my brain back.”
What the brochures undersell
Honest accounts keep the ledger balanced: wearing two devices with their sites, adhesives, and skin real estate; sensor warmups and dropouts at inconvenient moments; algorithms that handle steady days beautifully and need human help for pizza, sprints, hormones, and illness; and the learning curve of trusting the loop enough to stop overriding it — the habit veterans say takes months to unlearn. Cost and coverage battles appear in nearly every account.
When the work itself becomes the problem
Automation reduces the arithmetic; it does not end the relentlessness, and the accounts are honest about the collapse that can follow years of it — alarms nobody can silence, appointments approached with shame, the quiet decision to stop looking at the numbers for a while.
